More Info about WTM Visit

One of your rights as a participant in Medicare Part B Health Insurance is a no-cost “Welcome to Medicare" preventive visit. You can get this introductory visit only within the first 12 months that you have Part B. This visit includes:

  • A review of your medical, family and social history related to your health

  • A review of risk factors which may affect your quality of life and health

  • Height, weight, and blood pressure measurements

  • Calculation of your body mass index

  • A simple vision test

  • A review of your potential risk for depression and your level of safety

  • Information about creating advance directives (end of life treatment preferences)

  • Education and counseling about preventive services (screenings, shots, and referrals

  • A written plan letting you know which other preventive services you need

In some ways, this visit is more “clerical” than a typical office exam, with less emphasis on diagnosis of new disease or therapy of existing conditions. Because of your visit you will have more information about Medicare services, and your electronic medical record will be brought up to date with documentation regarding your specific medical status and provide a baseline for your future care. Thank you in advance for taking the time to complete this information.

Your complete answers to the questions are vital in assuring that you receive the full benefit of the Welcome to Medicare Preventive Visit. It is our sincere hope that the information obtained today will empower and enable you to engage in self-management and wellness, promoting specific activities such as any necessary weight loss, physical activity, tobacco-use cessation, fall prevention, and improved nutrition.

Please answer each section completely to your best ability and ask us if you need additional assistance.

If you have an existing electronic medical record in our practice, you will receive a printout of your past medical history, family medical history and list of current medications.
